Should you take a day trip to Sintra or Cascais?
I'd choose Sintra for palaces, gardens and a full sightseeing day, or Cascais for the coast, seafood and fewer reservations. For an easy alternative to Sintra, take the Cascais train from Cais do Sodré and leave the afternoon flexible. Choose Sintra when a palace is the reason for the trip: travel from Rossio, reserve Pena’s timed entry, and limit the day to two major sights. Cascais fits better after heavily scheduled city days or when the weather favors the water.
When should you choose Sintra?
Sintra’s best first route combines Pena Palace with one second monument and the old town. Pena uses timed palace entry and does not tolerate lateness. Transport between sights consumes real time.
The complete Sintra day trip covers the train, bus and two-sight limit. This is not the day to improvise five palaces.
When should you choose Cascais?
Cascais works without a monument schedule. Walk the center and waterfront, continue toward Boca do Inferno, then have a long fish lunch or beach time. The official visitor guide describes the town as about 40 minutes from Lisbon by the CP line from Cais do Sodré.
It is a better reset than a checklist. Wind, water temperature and weekend crowds still matter.
Which Lisbon station should you use?
Sintra trains commonly leave from Rossio. Cascais trains leave from Cais do Sodré. Both are part of the Lisbon urban network, but they run on different lines. Check the CP journey information for the date.
Do not plan to decide only after arriving at one station. Choose the day trip before breakfast and travel from the correct side of central Lisbon.

When to skip both
Skip the day trip when you have only two or three full days and still want Alfama, Belém, Chiado and Príncipe Real. Lisbon is not a gateway you must leave immediately.

Should you combine Sintra and Cascais instead?
I would combine them only for one Sintra monument followed by a short coast visit. If the point is Pena plus another palace, choose Sintra for the whole day. If the point is a beach afternoon and lunch, choose Cascais. Putting both names on an itinerary does not give you two full day trips.
The combined-day guide separates the palace approach, lunch and cross-country transfer. Cabo da Roca is the first detour I would cut. The palace reservation should determine the morning; the coast is what remains afterward, not another fixed appointment.
